About Virginia Schedule HCI
Virginia Schedule HCI is used when you voluntarily consent on your income tax return to share certain tax information for a health coverage eligibility review and would like to be contacted. The schedule lets you choose the state agencies that receive the information and provide your preferred contact method.
The 2025 PDF contains one page with brief instructions, agency selections, contact fields, and space for dependent information. The agency choices cover medical assistance through the Department of Medical Assistance Services and Department of Social Services, or coverage through the Virginia Health Benefit Exchange. Enter the requested information for yourself, your spouse if filing jointly, and your dependents.
Read the facilitated health insurance enrollment guidance in the Form 760 instructions or Form 760PY instructions, as applicable. Indicate your consent on the return, complete Schedule HCI, and enclose it with the return. This is a voluntary information-sharing option.

No. Schedule HCI accompanies the voluntary consent option for sharing information to help determine health coverage eligibility. Complete it if you give that consent on your return and want to be contacted. Follow the consent instructions for your return before attaching the schedule.
Which agencies can I select on Schedule HCI?
The schedule offers a selection for the Department of Medical Assistance Services (DMAS) and Department of Social Services (VDSS) to determine eligibility for medical assistance. It also offers a selection for the Virginia Health Benefit Exchange (VHBE) to determine eligibility for coverage through Virginia's Insurance Marketplace.
How do I choose my preferred contact method?
Fill in the appropriate oval and provide the requested email address, daytime phone number, or mailing information. The schedule includes a separate field for your spouse's email address. For contact by mail, enter an address if it differs from the address on page 1 of your Virginia return.
What dependent information does the schedule request?
The 2025 schedule has space for up to ten dependents. Enter each dependent's first name, last name, and birth date in the fields provided.
Where can I find the consent instructions?
The Form 760 and Form 760PY instructions explain facilitated health insurance enrollment, the information that may be shared, and where to indicate consent on the return. Schedule HCI includes brief instructions for choosing agencies and completing the contact fields. Enclose the completed schedule with your individual income tax return.
